Please use the Elmore Office Suites entrance.
Located on the second floor at top of the staircase, or if needed the elevator is located just inside the doors.

On your first appointment:
- Please wear loose fitting clothing. You may consider bringing shorts or a sleeveless shirt depending on your injury location.
- Please bring your health insurance card(s) or policy information.
- Please bring the referral or prescription for PT if given to you by your physician. This may have been mailed or faxed to Southwind Physical Therapy.
